Becoming a Patient

Circle Health Services’ mission is to provide quality health care and related services free of charge to those who lack appropriate alternatives, including the uninsured, underinsured and those individuals with coverage through Medicaid or Medicare.

The first step to becoming a patient is to call (216) 721-4010 to schedule a registration appointment. All new patients must complete the registration process prior to their first medical, dental or behavioral health appointment. To register as a Circle Health Services patient, please bring the following documents with you to your appointment:

  • Valid state ID
  • Proof of income (pay stubs, tax forms, etc.) – We determine your income by looking at the ways you are supported, even if you are not working.
  • Insurance card, if relevant
  • Proof of family size (birth certificates of children, tax forms, etc.)

All information presented to Circle Health Services is kept confidential in your health record. You do not have to be a United States citizen to receive care at Circle Health Services.

Translation services are available for non-English-speaking individuals. Please request a translator at the time you schedule your appointment.

Circle Health Services is committed to providing high quality care to our clients, but we understand that you have other needs as well.   At your registration appointment, you will have the opportunity to meet our Financial Counselor as well as members of our Social Service team.  Through the assistance of a short questionnaire, the Social Service Worker will gather information regarding your needs and connect you with departmental referrals within Circle Health Services as well as resources in the Cleveland community.  From basic needs to workforce development, we have the resources to connect you, and by doing so, we at Circle Health Services hope to support you in reaching your healthy living goals.  Please call to schedule a new patient registration appointment at (216) 721-4010 or (216) 707-3484.
